MIPS Hardship Exemption for “Small” Practices

Clinicians who are enrolled in Medicare are required to participate in the Merit-based Incentive Payment System (MIPS). Under MIPS, performance on four (4) components determines updates to the physician payment in subsequent years (2019 performance impacts 2021 reimbursement). Your participation may be linked to an ACO or physician network which handles Quality, Improvement Activities and Cost. The fourth required component is called Promoting Interoperability (PI), formerly known as Meaningful Use, and is often the practice’s responsibility. As a small practice, it can be challenging to perform well on all the PI measures. In recognition of this fact, CMS has created a hardship exception application for small practices (< 15 clinicians) that would exempt successful applicants from reporting on the PI component.
